01Is your MCS-150 on file and updated within the last 24 months?

Your biennial update is due on a schedule set by the last two digits of your USDOT number, whether or not anything about your business changed.

02Does every driver hold a valid CDL for the vehicle they operate, with no disqualification in effect?

A lapsed, suspended, or wrong-class CDL makes the driver unlicensed for that vehicle, and using a disqualified driver is its own violation. Both are single-occurrence automatic failures, so one driver on a bad license is enough.

03Do you have the required minimum levels of liability insurance in effect?

Coverage being in effect is what the regulation requires. A policy that lapsed, or a filing that never reached FMCSA, both leave you operating without it.
